Health Certificates
Pawsitive Pet Health Certificates will collect your pets health information and evaluate what Veterinary services are needed in order to qualify for travel.
You will need the following information:
- Your Veterinarians information and all Veterinary Hospitals your pet may have received services from in the last 3 years.
- The date you are scheduled to travel.
- The destination state or country you will be traveling to.
- The Airline carrier, departure airport, and arrival airport. As well as any layovers you may have.
- Your pets information.
